Second Family Care Team®

The SECOND FAMILY CARE TEAM® project serves chronically ill and frail adults by providing reassurance that, though perhaps alone and physically limited, they are not abandoned and their needs not overlooked.

Second Family Care Team® members (the title suggests the vision of a Care Team® as a "second family" for frail adults and their caregivers) keep these adults connected to the community and support them, as needed, with companionship, and practical support, such as errands, shopping, preparing meals, and occasional chores.

Other forms of assistance may include assisting care partners to keep track of doctor visits, medication schedules, understand what other community services exist, and how to access them. The Second Family Care Team® provides caregivers with encouragement, hope, respite, emotional support, and physical assistance.
