Interfaith CarePartners serves persons with Alzheimer’s disease or another dementia and their caregivers in two unique ways through its Alzheimer’s Care Team® model:
Most teams serving care partners with dementia conduct once or twice-monthly activity times onsite, at their congregation campuses. Alzheimer's Care Team - Gathering Place meetings combine physical movement, socialization, music, and other memory-stimulating activity in a safe and comfortable 3.5-hour time block, enabling caregivers to attend to their personal needs without worry. A professional staff member attends each Gathering Place to provide expert support and assistance to the volunteers. To view a complete map of all zip codes and locations click here.
Volunteer Alzheimer’s Care Team® members regularly are in care partners’ homes, engaging persons with dementia in conversation and appropriate activities, and giving their full-time caregivers—who are often overwhelmed and isolated—much-needed worry-free breaks.
